HANDS Ongoing Home Visitor
The Woodford County Health Department is accepting applications for contractual employment for a full time HANDS Ongoing Home Visitor

​

General Duties include: 

  • Provides visits to eligible clients through home visitation and/or telehealth visits to
    families.

  • Work will require travel to family’s homes or within the communities served.

  • Keep accurate logs of service visits, schedules and charting, and completes paperwork
    as set forth in HANDS program policy and guidelines.

  • Provides outreach and resources for the HANDS program.

  • Conducts, plans, and schedules HANDS visits consistently in accordance with guidelines
    of the program.

  • Facilitate obtaining resources through local community agencies for families.

  • Discuss results of HANDS screenings or any developmental concerns and makes
    referrals, as appropriate.

  • Distributes HANDS resources and materials to families and community partners.

  • Promote the HANDS program at events, festivals, etc. to educate families on the
    program as well as develop partnerships to expand outreach in the community.

 

Minimum Education, Training or Experience:
High School graduate or GED.

Special Requirements: (Age, Licensure, Regulation, Etc.)  Must have a current driver’s license, reliable transportation, and current vehicle insurance. Must be 18 years of age.

 

Salary:  $19/hour. Negotiable with additional education and experience. Agency Benefits provided.

Project Coordinator (Full-time)

Youth Engagement Project (YEP!) is a substance use prevention program that provides engaging activities for youth during out-of-school hours to reduce risky behaviors.

​

SALARY: $48,750 to $58,500 per year plus benefits, salary commensurate with experience

​

POSITION WLL REPORT TO: YEP Executive Board and Public Health Director or designee

​

EDUCATION: Bachelor’s degree, preferably in human services

​

EXPERIENCE: One year of experience in a supervisory or management capacity in a community health agency or youth-serving agency that would demonstrate the necessary knowledge, skills, and abilities to implement a youth substance use prevention program.  Experience expanding outreach to diverse youth populations preferred.

​

DUTIES: Responsible for coordinating all activities as required for effective implementation of the YEP youth substance use prevention model in Woodford County including:

​

  • Planning, organizing, and directing a full range of activities that promote healthy alternatives to youth substance use.

  • Developing digital and print media campaigns to raise awareness and generate interest.

  • Maintaining social media accounts that promote healthy alternatives to drugs, alcohol, and other substances.

  • Organizing, publicizing, and hosting Parent Cafes as well as Lunch and Learn events.

  • Overseeing the creation of a diverse Youth Council.

  • ​Promoting equity and inclusion in all YEP endeavors.​

  • ​Collaborating with Woodford County Health Department, YEP steering committee,
    YEP activity providers, students, and family members, as well as WCASAP coalition members, sector representatives, and stakeholders.

  • Recruiting, training, and evaluating youth-serving Activity Providers.

  • Developing and promoting mutually beneficial relationships with other agencies.

  • Building relationships with community partners to expand the coalition.

  • Collaborating with fundraising committee.

  • Preparing quarterly reports to be presented to DFC Evaluating Team, Board of Health, ASAP Coalition
