Fees for Services
The Health Department charges fees for certain services. Some of these are a flat nominal fee of $5.00. Some are a flat fixed charge that covers the actual cost of the vaccine or lab test plus the nominal fee to cover staff time. Some are based on income and are assessed via a sliding scale (the Uniform Percentage Payment Schedule or UPPS), i.e., the more you make, the more you pay for a service up to 100% of the cost. For these services, we MUST obtain an income. If you choose not to provide your income, you will be charged the full fee for the service.
The UPPS was adopted by the Board of Health at it's October 2004 meeting and took effect January 4, 2005. The Woodford County Health Department was, at that time, one of 6 health departments across the Commonwealth that had not adopted the UPPS. The Board felt that charging those who could better afford to pay for their services was a good way to stretch scarce resources. The Board realizes that there is a Public Health Tax in Woodford County. They also realize that the tax only covers about one-fourth of the cost of operating the health department. The fact that you may pay the Public Health Tax is not an argument for failure to pay for services when you get them.
No one will be denied service because of inability to pay. However, payment is expected at time of service. If you cannot pay at that time, arrangements will be made for you to pay weekly, bi-weekly or monthly until you have paid your bill.
